PAYMENT & FEES

Dr. Fowler does not accept any form of insurance and is strictly a “fee-for-service” provider. Initial consultations are $325 and all follow-ups are $150/visit. (Please note that our point-of-sale software does apply a $5 transaction fee.)

A super bill (medical receipt) can be given upon request after payment for each visit. If applicable, these receipts may be submitted to your insurance company for reimbursement. NOTE: Reimbursements vary depending on the plan and/or insurance company you use. Clients are encouraged to contact their insurance carrier to inquire about reimbursement rates for Dr. Fowler’s services as an “out-of-network” provider.

Palmetto Psychiatric does not provide assistance in contacting insurance carriers on clients’ behalf.
